If there is a form you want people to fill out when they signup, create it using this page. For example, this page is currently showing a simple form that allows someone to enter a comment when she signs up.

If you want the same form for most activities of an event, you would make it the event's default form. If you have a special form for certain activities, you would go to Manage Activities, select them, and click on any Update Activity icon.
